Integrated Health Home

WHAT IS IHH?

An integrated health home (IHH) is a team of professionals working together to provide whole person, patient centered, coordinated care for adults with serious mental illness and children with serious emotional disturbances.

IHH ELIGIBILITY

  • Enrolled in Medicaid/Title 19

  • An adult with a Serious Mental Illness (SMI)

  • OR

  • Children and youth with a Serious Emotional Disturbance (SED)

WHY DOES IT MATTER?

Behavioral health plays a critical role in an adult and child's ability to maintain good physical and social health.  Studies show that adults and children with mental health challenges often experience worse health outcomes than their peers.

SMI DIAGNOSES

Schizophrenia, schizoaffective disorder, bipolar disorder, major depression, or other serious mental health conditions that cause significant impairment in daily functioning.

SED

A diagnosable mental, behavioral, or emotional disorder of sufficient duration to meet diagnostic criteria specified within the most current DSM manual that results in a functional impairment.
